Abstract: For needs of closed loop management of hospital diagnosis and treatment, weak link of health follow-up for patients after hospital was analyzed, and function process and framework of follow-up system for hospital discharged patients were designed, which adopts XAMPP integrated environment, introduces MVC framework, separations model, control and view, builds network environment of online follow-up system, and implements common follow up interface and background maintenance by using PHP. System is beneficial to humane care and rehabilitation of discharged patients, improve closed loop management after hospital, improve patient satisfaction, and easy maintain and have high reusability.
